In an effort to safeguard your PHI (Protected Health Information) Any Path Physical Therapy has implemented HIPPA and HITECH compliant software for you to be able to fill out using your computer and send your completed forms digitally.
Once you make an appointment for your first visit (physical therapy evaluation) you will receive an email with instructions on how to get into the patient portal. On the patient portal you will be able to fill out the intake paperwork and transmit it with encryption.
If you need any additional forms click here for paperwork such as a “permission to share health care with” form to coordinate your care between health care providers or a “personal representative form” to add a family member, spouse, or guardian etc as a contact for your health information.
Any Path Physical Therapy LLC can check your insurance benefits as a courtesy and most importantly perform a thorough chart review so that during the 60-minute new patient evaluation or new condition evaluation there is plenty of time to listen to your concerns, your goals for physical therapy, evaluate your condition, and potentially start treatment if physical therapy is right for you.
