Patient Portal – Online Scheduling

There are four portals below to choose from so you can schedule online for both new and existing patients. If you get stuck or need help at any point along the way please call (603) 316-4942 between 7 am – 11:30 am and 12:30 pm – 7 pm for assistance in scheduling.

Traditional Portal

This link is the general portal login link containing the clinics Portal ID. This can be used for new patients to register as well as existing patients to log back in.

Self Service

This link is used for new patients to go through a wizard and pick their appointment first and then register and fill out intake. If you are using a mobile device the page may look a little jumbled, while we fix this, you can still get to the self service portal just scroll all the way down to the right until you see the buttons “Existing Patient”, “New Patient”, and “Contact Us”. If you have trouble, you can always call (603) 316-4942 to schedule.

This link is a form that allows a patient to create their account first and then schedule and fill out intake.
